Village Medical’s Houston network aims to make primary care accessible to Houstonians in their neighborhoods. They hire physicians and staff from the same communities where their clinics are located across the city.

“If you live and work in a certain area of the city, you understand the nuances of the neighborhood and the population,” Dr. Geracci said. “That local insight helps our providers and staff relate to their patients better, understand the social factors influencing their health, and anticipate their health care needs to offer a more tailored plan of care.”

This connection to local communities goes beyond convenience. It allows providers to understand cultural, socioeconomic and lifestyle factors affecting health. By knowing the population they serve, Village Medical can offer practical, meaningful, and more impactful care, helping patients stay healthier and more engaged in their own care.

Coordinated team-based care

For many patients, health care can feel fragmented—multiple specialists, conflicting prescriptions and uncoordinated tests.

Village Medical addresses this by building closely collaborating care teams.
