Memorial Hermann Southwest Hospital introduced the new Adult Care for Elders unit, a division of the hospital designed to address issues specific to older patients, on Sept. 19.
“Collaboration among our physician, employee and community partners has made our ACE Unit possible, and we truly believe it will make a big difference in the lives of our senior population,” said Gillian Alexander, vice president and chief nursing officer for Memorial Hermann Southwest Hospital, in a press release.
The goal of the unit includes the maintenance and improvement of patients’ health, a decrease in the need for a transfer to another facility, getting patients home sooner, the decrease in adverse events, an increase in education about and connection to community resources, and the reduction in overall readmissions, according to Malisha Patel, senior vice president and CEO of Memorial Hermann Southwest Hospital.
