Dell Children's at Home, the pediatric hospice program at Dell Children's Medical Center, is transitioning from home-based care to an outpatient clinic setting, a spokesperson told Community Impact on Oct. 30.
What's happening
Care will be centralized at Dell Children's Comprehensive Care Clinic, located on the second floor of Austin ISD's Rosedale School campus. It serves children 17 and younger with complex medical needs, according to its website. The clinic setting offers specialized symptom management, coordinated clinic appointments and daily telephone check-ins.
"Centralizing care within our Comprehensive Care Clinic seamlessly connects families to our pediatric palliative care physicians, case managers, and social workers all in one location," the spokesperson said.
Complex care referrals are already underway, and individualized transition plans have been created for each child.
