Co-founders Rick Reeder and Bart Arnold launched the company after navigating their own family members' care transitions and discovering how much they didn't know.
The wake-up call for Reeder came when his in-laws experienced a sudden medical crisis. They were in a Type A assisted living facility, meaning minimal assistance was offered, forcing an abrupt and stressful second relocation to a Type B facility with higher care.
“If we had known about Type A and Type B facilities, we would have moved them to a Type B facility initially,” Reeder said. “There’s a lot of information about situations like this that people have no idea about, and they don’t know what questions to ask to get that information.”
For Arnold, the realization hit when his 91-year-old father could no longer safely live alone on his remote 37-acre property due to driving limitations.
Suddenly, Arnold was juggling multiple tasks: selling the real estate to fund his father's care, packing multiple buildings, establishing power of attorney and trying to decipher which facility fit his father's needs—all while managing his own career and family.
“I realized how overwhelming this task is,” Arnold said.
Driven by these struggles, the co-founders launched True Care Transitions earlier this year to serve as a holistic guide for other families.

Rather than leaving families to piece together a solution on their own, googling one problem at a time, True Care Transitions connects families with vetted professionals across every dimension of a senior transition.

How does the process work?
True Care Transitions starts with a detailed questionnaire to understand each family's unique situation, including care needs, timeline, financial circumstances and more. From there, the team makes personalized referrals and provides warm handoffs to the specialists who fit those needs.
Consultation and referral guidance is free for families. Individual service providers charge separately for their work when applicable.
Protecting vulnerable seniors from scams
Beyond coordination, the founders had another concern in mind: seniors in transition are frequent targets for fraud. Arnold encountered this firsthand while helping his in-laws.
“I started getting calls from people who I quickly realized were scam artists,” Arnold said. “Being able to offer people vetted professionals and vetted companies, [most of whom we have worked with], that aren’t going to take advantage of seniors and families is important.”
What's next for True Care Transitions?
Reeder and Arnold are looking to expand their services, potentially adding Medicare and Medicaid guidance, while staying focused on delivering immediate relief to families.
“It’s a great feeling when people call us to say, ‘I have no idea how to do this,’ and we can help them,” Arnold said. “We can feel their relief over the phone because they realize they’re not swimming by themselves in the middle of the ocean.”
