Primary care helps patients turn motivation into realistic goals. Providers work with them to create personalized plans that are easier to maintain year-round by reviewing health history, lifestyle habits and individual needs.

Follow-up visits track progress, adjust goals and provide accountability, while connections to nutrition, fitness or specialty resources offer additional support.

Primary care physicians provide practical guidance and personalized support for patients wanting to exercise more, eat healthier or lose weight. Common recommendations include:
  • Balanced meal guidance using MyPlate
  • Low- to moderate-intensity exercise options
  • Referrals to nutritionists, personal trainers or fitness specialists
“Even small changes—like walking more or choosing healthier foods—can make a big difference over time,” Requena said. “We encourage patients to start safely and set realistic goals, which makes it easier to overcome common barriers to weight loss, such as feeling overwhelmed or unsure where to begin.”

Primary care physicians help patients turn motivation into sustainable results by combining education, achievable goals and access to resources.

Staying active with chronic conditions

For patients with chronic conditions such as diabetes, high blood pressure or high cholesterol, primary care physicians help balance disease management with new wellness goals. UTMB providers coordinate care with specialists and connect patients with educational programs.
