Walking into The Reserve at Richardson, it’s clear this is not a typical skilled nursing facility. Built in 2017 and designed for comfort and clinical excellence, the community offers a warm, modern environment where short-term rehabilitation and long-term care residents receive personalized support.

“As a 129-room skilled nursing facility, we can meet residents wherever they are in their care journey,” Executive Director Roy Hurst said. “Our skilled residents are downstairs, working through therapy seven days a week. Upstairs, we care for long-term residents—people who have made this their home.”

The Reserve at Richardson offers a comprehensive continuum for individuals recovering from hospital stays, surgeries, injuries or navigating long-term health needs.

Patient-centered care at every step

The Reserve’s clinical approach focuses on understanding each resident’s needs and helping them regain independence.

Care includes medication administration, wound care, strength training, mobility support and daily living activities. Every detail—like identifying a resident’s strongest foot—is assessed to maximize safety and support successful outcomes.

“We want residents to be as independent as possible,” Hurst said. “If there’s a device, therapy method or accommodations that will help, we identify and use it safely.”

Because the therapy team is on-site, sessions can be scheduled around each resident’s natural rhythms. If someone performs best after breakfast, therapists adjust. This level of personalization is difficult to achieve in a hospital.



Why skilled nursing?

Many residents arrive after extended hospital stays, strokes, surgeries or major illnesses. Hospitals stabilize patients, but they aren’t built for mobility, endurance building or relearning daily tasks. Skilled nursing bridges the gap.

This middle step allows individuals to rebuild strength, receive hands-on training for new medical needs—such as managing an ostomy—and reestablish routines before returning home.

“Being here gives people independence with a full team watching over them,” Hurst said. “We’re educators. We teach you what you need to keep improving long after you leave.”
