When mobility, strength or independence suddenly slip away, the road back can feel overwhelming. In those moments, a patient’s rehabilitation care quality becomes a key predictor of their recovery speed and extent.

Exalt Health Rehabilitation Hospital in Conroe steps in. Built around exceptional care, compassion and a commitment to excellence, it helps patients regain independence safely, efficiently and with dignity.

Where recovery becomes reality

Exalt Health is a 40-bed, single-purpose inpatient rehabilitation hospital designed for patients needing intensive therapy after a major medical event. Unlike general hospitals, rehab facilities help patients regain independence, mobility and daily function, rather than just stabilizing medical conditions.

Patients come to Exalt Health for structured care and specialized therapies for safe and efficient recovery. Those who benefit most are recovering from:
  • Stroke
  • Amputation
  • Falls or fractures
  • Brain and spinal cord injuries
  • Pulmonary rehab care
  • Spasticity management

Recovery support services

At Exalt Health, patients receive personalized, intensive therapies to regain independence safely and efficiently. Inpatient Rehab services include:
  • Physical, occupational and speech therapy
  • Wound care
  • Hemodialysis
  • Nutritional consults
  • Pulmonary, neurology and orthopedic programs
Patients participate in three hours of daily structured therapy delivered with specific goals to meet their individual needs. Outside these sessions, nurses and staff work with individuals throughout the day, helping them reinforce progress made through daily activities in real-life settings.

“Patients must participate in about three hours of daily therapy, but what they do during the remaining 21 hours is just as important,” Chief Operations Officer Jeff Crawford said. “Every movement outside therapy, from walking to the bathroom to sitting at the dining table, is an opportunity to practice skills, build strength and reinforce progress. Our staff is there around the clock to guide and encourage patients, ensuring recovery continues outside formal therapy, leading to better outcomes.”

A facility focused on safety

Exalt Health’s hospital is designed to enhance recovery. It creates a bright, calm environment that encourages movement and independence with floor-to-ceiling windows, wide hallways, open therapy gyms and arranged patient rooms.

Every detail is built with safety in mind. Line-of-sight nursing stations, strategically placed alarms and clear pathways allow patients to move confidently while staff can respond immediately.

Exalt Health creates an environment for faster, safer and more confident patient recovery by combining state-of-the-art facilities with comprehensive therapies and continuous support.

Better, data-backed outcomes

Unlike facilities relying on internal reporting, Exalt Health highlights outcomes directly from Medicare’s public database. Recent six-month data show:

Shorter average stay: Exalt Health patients spend an average of 11.04 days in the hospital, compared to the national average of 13.29.

Fewer falls since admission: At Exalt Health, 3.29% of patients fell, compared to a 4.59% national average.

Higher percentage of patients return to their communities: At Exalt Health, 77% go home or to a community-based setting, compared with 74.16% nationally.

“We take on higher-acuity patients, and our average age is older than the national average,” Crawford said. “Through focused, around-the-clock care and a facility designed for recovery, we get more patients back home faster and safer than the national average. That’s the difference intentional care makes.”

A local option for Montgomery County

Exalt Health’s Conroe location was chosen with patients and families in mind. For Montgomery County residents, having a nearby, high-quality rehab hospital means shorter travel, easier family visits and a more supportive recovery experience.

“Being in the community where our patients live allows family involvement,” Crawford said. “It strengthens continuity of care, because local physicians can follow their patients from the acute hospital to our rehab facility. That local connection makes recovery feel safer, more personal and more successful.”

Exalt Health in Conroe offers accessible, community-centered rehabilitation care without compromising expertise, safety or outcomes.
